This episode is part 1 of a 2-parter. The second half will be the S13 premiere.
The following spoilers courtesy of AEB: I did not write them so the opinions expressed throughout are that of the original poster.
Steve and a prison friend have planned an escape from the ER. They get in a fight and are brought in for treatment, accompanied by a female EMT who's also in on the plan. Steve's friend and the EMT are to be name actors. Several patients/bystanders are caught up in the chaos, including a kid, a couple of elderly ladies who've come from church to pray for someone, etc.
A farewell tape is being made for Morris.
Early on, Sam has a new EMT "observing" her, called Mary. She seems a bit dense and is getting in Sam's way. In a trauma with Abby & Luka, Sam is not her usual efficient self
They are treating a man who has been found slumped in his car. His church friends say he'd been missing for a week. Abby & Sam treat him first, but can't work out what's wrong - Luka turns up and works out its something to do with his medication.
After, Sam apologises to Luka. He's says he expects more of her - and if the EMT is a drag, Sam should "get rid of her". Mary (EMT) describes Luka as a "gloomy gus" who "needs to lighten up".
The church friends are surprised that Abby is having a child when she doesn't appear to believe in "a higher power".
Ray has been trying to contact Neela to see how she's doing, but Pratt says to give her some space.
It's Morris's last day. When a couple of prisoners are bought in by two sherrifs (steve and a guy called Rafe), Morris says he's not treating them on his last day - Ray can have them.
We see Neela at Gallant's funeral. Pratt is the only ER staffer there. Neela is not at all moved by Col Gallant's words, while everyone else including Pratt is. An instruction says that among the mourners is a young woman in a wheel chair - "looks like Val". No word if Pratt says anything to her...
Jerry is playing a computer game with a young boy - who's beating him.
It seems Steve & Rafe had a fight because Rafe was trying to steal Steve's smokes. Sam is treating both of them. Mary turns out to be someone else entirely - she drugs the sheriff guarding Rafe by placing a cloth over his mouth and comes in to Sam/Steve's trauma room and puts a gun to the neck of the other sheriff.
Don't know where Luka is - the last mention of him is something about hearing his breathing and...
Rafe, Sam, Steve move out... Sam seems normal, until she calls Abby "Abigail"...
There's a shoot out. Jerry is indeed shot in the chest.

The shootout/escape is in the last third of the episode. Abby takes a "nasty spill" and cuts her head, but still manages to save the day and take charge afterwards. Jerry shields a child in the chaos and gets shot in the chest. Steve and his friends take Alex and Sam hostage and escape in a van.
It's Morris' last day at work.
Neela's "stony-faced" at the graveside service, while Gallant's dad gives a eulogy. Pratt appears to be the only other staff member in attendance.

Apparently the current wisdom on the Random Spoilers thread (I think - I'm losing track of where things are) is that it is Luka, who's been injected with paralytics by the paramedic in cahoots with Steve and Rafe, based on what Elizabeth saw during her time on the set. It would certainly explain Sam's panic, and her urgency to intubate, to the degree she's doing it without gloves. She's usually so calm under pressure, and I can't see this degree of panic were it a cop. It also would seem to fit the "last thing we hear is Luka breathing" spoiler for the end of Act III, is it? Sam gives Abby a clue, and Abby probably puts it together and finds Luka.
Besides, how much fun it it to have it be the cop rather than Luka? If it's Luka - that's where the drama is. If he's just been given a paralytic, he'd be awake but paralyzed. I'll be interested to see how long it takes for him to get back on his feet and take charge.
